MedInternet (MI) is a project of a global, online automated healthcare system, operating on the basis of a sequential multi-step algorithm corresponding to the algorithm of a real medical process.

It is a future internet medicine giant, designed for global automation of the practical work of the professional healthcare community, as well as an online system for managing processes at different levels from a single center using uniform standards.

Positioned at the intersection of modern medical knowledge and information technology, MI is able to concentrate, unite and manage online global healthcare system, reflecting all areas and stages of its work. The project envisages that MI will be capable of the following:

  • accurate diagnosis including identifying hidden diseases of patients
  • search and select the optimal treatment for the diagnosed disease in each specific case
  • automatically generate and update Global Online Electronic Medical Record (GOEMR) of each patient, in the process of a doctor’s work, in a single format, reflecting all basic medical indicators of the patient

In what order and sequence will MI work

Algorithms of MI

Doctors around the world will be offered an online web resource for use in everyday practical work on diagnosing and treating patients - an IT tool, with the option of working in a selected language. Having accessed this Internet resource through their personal computers, medical practitioners will have the opportunity to register their online-virtual medical offices and begin working with patients through the MI system, without disrupting their usual routine. The difference will be that there will always be a doctor’s personal computer, connected to the MI, between the doctor and the patient.

Initially, the doctor will enter the patient’s personal data in a given format, then select anthropometric and anamnestic data from the databases.

Next, the doctor will select from the appropriate databases and enter the patient’s existing pathological signs, symptoms, laboratories and other medical pathologies into the GOEMR using a special search mechanism and the MI diagnostic module will find accurate diagnosis in each specific case. The diagnostic process will take place on the basis of a predetermined medical knowledge base.

  • The patient’s GOEMR will be comprised of the following sections:
    • anthropometric indicators
    • anamnesis vita
    • diagnostic indicators
    • the diagnosis
    • optimal treatment prescribed by a doctor (selected from the GOEMR database)
    • the result of treatment of the disease
    • results of various tests and studies of the patient’s body
  • All these GOEMRs of a single form and structure will be concentrated in a database and as the main information structure of MI, will form the Global Medical Big Data. The difference between GOEMR and other EHRs is that it will be constantly available online to the doctor, patient and insurance company.

The implementation of MI requires the initial creation of the following medical knowledge base and special software:

  • medical knowledge bases, with a detailed digital description of existing diseases according to a predetermined form and structure (descriptions of diseases with their symptoms and signs)
  • laboratory databases
  • databases of typical pathological conditions of the body, created and selected from the “anatomy + pathology” databases, with their description in digital format reflected in EEG, ECG, MRI, CT, PET, Echo and X-ray images and provided with a special search and identification mechanism

With the creation of the above, the following will become possible in automated mode:

  • creation of a system for automated diagnosis of diseases
  • automated formation and registration of patients’ GOEMR in a unified form and structure
  • creation of an automated system for selecting the optimal treatment from the GOEMR database in each specific case
  • online registration and concentration of all GOEMRs in the Global Medical Big Data
  • the capabilities of software analysis of Big Data of GOEMR to create a wide variety of clinical statistics and new medical knowledge

In what order and sequence will MI work

Algorithms of MI

The diagnostic process will take place on the basis of a predetermined medical knowledge base, including the following:

  • Digital description of existing diseases with their detailed symptoms, signs and possible anomalies in organs and parts of the body.
  • Patient’s personal medical parameters, such as anthropometric data and medical history (anamnesis vita). During the process of the doctors’ work with patients, this data will be selected from databases and reflected in the corresponding places of the GOEMR.
  • The condition of the organs and parts of the body will be selected from the anatomical and pathological databases according to the “organ + pathology” principle (e.g: "the right upper lobe of the liver" + "multiple, diffuse, cystic, lesions", etc.)
  • Optimal treatments, as they are formed and accumulated, will be automatically selected from the treatment section in the Big Data of GOEMR, according to the precedent principle: similar organisms, similar pathologies, similar treatments. Until the database of precedent treatments is sufficiently filled and formed, doctors will prescribe treatments based on their own knowledge and experience.
  • The information generated as a result of the above process will automatically generate and save the GOEMR of each patient in the doctor’s account, the patient’s gadget, and anonymously copied into the GOEMR Big Data. The results of laboratory and instrumental studies for a certain date with corresponding comments and images will also be attached to the card.
  • As such, the patient’s GOEMR will be comprised of the following sections:
    • anthropometric indicators
    • anamnesis vita
    • diagnostic indicators
    • the diagnosis
    • optimal treatment prescribed by a doctor (selected from the GOEMR database)
    • the result of treatment of the disease
    • results of various tests and studies of the patient’s body
  • All these GOEMRs of a single form and structure will be concentrated in a database and as the main information structure of MI, will form the Global Medical Big Data. The difference between GOEMR and other EHRs is that it will be constantly available online to the doctor, patient and insurance company.
  • The system will also be able to detect hidden diseases and warn in advance about possible future diseases of the patient.

Taking into account its capabilities for performing tasks regarding the use of forms and methods of global online work, the volume and speed of information processing offered, and the automated creation of new medical and healthcare knowledge from them, it is safe to say that MI is no longer merely a platform of digital medicine, but of INTERNET MEDICINE with its own Artificial Intelligence, responding to challenges of the 21st century.

Being open and transparent system, MI will also facilitate direct communication of the entire global medical community (doctors, clinics, patients, insurance and pharmaceutical companies, medical schools, medical equipment manufacturers, as well as other participants in the global medical process) in a single information space using uniform medical standards.

The only steps required will be to connect to and register in the MI system.

Working with the MI is quite simple; as such, it will take place in parallel with the doctor’s usual work and will not require additional effort and time from them.

Constant updating of the database and development of New Medical Knowledge of MI

Thus, in addition to automated diagnosis and selection of optimal treatment, special software constantly analyzing the GOEMR, will be able to generate various clinical and healthcare statistics, as well as new medical information and create New Medical Knowledge. New Medical Knowledge will become the basis for the Knowledge of Evidence-Based Medicine. Medical science will be developed from the evidence-based medicine.

This will be comprised of a permanent Brain Center, which will consist of the following:

Summary:

  • The current level of information technology and medical knowledge makes it possible to create a single, online automated medical information system accessible to the entire global medical community, uniting the global medical process into a single whole.
  • This mechanism can be characterized as a unified, global, online automated information and management system for healthcare.
  • Despite being technologically ahead of its time in significant ways, MI doesn’t require any new knowledge or inventions - only existing computers, the internet, software and server capacity.
  • The transition to working under the MI system will be carried out by doctors and other users gradually, as they wish, with the acquisition of the necessary knowledge and skills during the test period of work.
  • To implement the project, it is necessary to create the following online digital medical knowledge bases and software:
    • databases of diseases with their symptoms and signs
    • database of laboratory tests with reference ranges
    • bases of pathological conditions of organs and parts of the body (anatomy + pathology)
    • database of anthropometric and anamnestic indicators
    • database of existing types and methods of treatment with their components
    • software search mechanism for diagnosing diseases
    • programs for automated selection of optimal treatment precedents from the GOEMR database according to the principle: similar organisms, similar pathologies, similar course of the disease - similar treatments
    • programs for the selection of drugs with connection to existing regional pharmacological databases
    • programs for the automated formation of patients’ GOEMR according to a single, predefined form and architecture
    • creation of special software for analyzing Big Data of the GOEMR and the formation of new medical knowledge of MI
  • Creation of special explanatory online instructions for doctors and other MI users in their working languages.
  • Necessary marketing.

The following work has been carried out so far:

  • More than 1200 diseases have been created and entered into the system, including more than ten thousand symptoms, signs, laboratory standards and instrumental pathologies.
  • Special administrative pages have been created for updating and editing MI databases.
  • An interface has been created for selecting the patient’s anthropometric indicators.
  • A database of patient history indicators and a sampling interface have been created.
  • A diagnostic interface has been created.
  • A draft pharmacological treatment module has been created, based on the “medical prescriptions from a doctor - electronic prescription for the patient” principle. As the GOEMR treatment base increases, this module will make a smooth transition to the automated selection of optimal treatment precedents.
  • A database of existing types and methods of treatment accepted in modern medicine has been created.
  • A brief database of treatment results has been created.
  • The MI database currently exists in two languages (English and Russian).
  • A draft demo version of MI has been created, including GOEMR formations.
  • An interface has been created for linking the MI diagnostic module to Google Images.

Upcoming work required to launch the MI:

  • Increase of the number of existing diseases (about 3000).
  • Assessment of importance (in%) of individual signs and symptoms in various diseases for their further accurate computer calculation as the MI works.
  • Expansion of the “anatomy + pathology” database.
  • Preparation of anatomical and pathological silhouette of signs and symptoms.
  • Creation of a database of side effects of pharmaceuticals.
  • Creation of a database of drug interactions.
  • Creation of an interface for analytical and statistical analyzing.
  • Increase of the number of languages.
  • Increase and adjustment of all databases.
  • Improvement of the structure of GOEMR.
  • Preparation of online instructions for users.
  • Necessary online marketing.
  • Creation of an application for gadgets “My GOEMR”.
  • Increase of the database of laboratory tests.

Completing the entire scope of the above work requires a team consisting mainly of doctors and highly qualified programmers, numbering at 15-20 full-time employees.

The costs of creating MI are mainly the salaries of doctors and programmers. According to my estimates, carrying out the above work at a good level will require a sum in the range of 800-900 thousand euros and will take no more than one year of time.

With all of this being said, for the implementation of the present project, which aims to respond to the challenges of 21st-century medicine and healthcare and directly concerns the health of every person, I would like not only to seek investment, but also establish a fruitful partnership. After all, we have very serious and multifaceted work laying ahead of us, aimed at creating a Global Internet Medicine without borders.
